How can I make an appointment?

Please carefully read over our Process before scheduling. Once you believe you are a patient that might be eligible, you may contact our office at (000) 000-0000 between the hours of 8am-4pm, Monday-Friday. Office visits will only be scheduled for Mondays on a first come-first serve basis.

How much does a consultation cost?

Please refer to our Fee List.

Does Insurance cover this?

No. Insurance does not cover your office visit to obtain a THC card. However, if you need multiple visits before becoming eligible for a THC card, your insurance may cover those visits. We do not guarantee insurance coverage for any visit or procedure and it is the patient's responsibility to work with their insurance company to determine coverage.

Where is the clinic located?

We are located in North Griffin at 1669 North Expressway, Griffin, GA 30223 inside of Internal Medical Center.

Do I need to be referred by a doctor?

You are required by law to provide us with your medical records before your office visit. This includes records from your primary care physician, pain management clinic, or any other medical professional who is currently treating you for a medical condition. You can find the document [here] to download and fax to your doctor's office(s).

If you are currently a patient of Family Medical Center or Internal Medical Center, our providers have access to your medical records and you will not need to request medical records specifically for your visit to SMT. Please notify the staff that you are a current patient while scheduling your appointment.

What payment options do you offer?

Office visits for THC Cards may be paid with cash or credit card. We do not offer payment plans. Payment is due in full at time of visit.
